Job summary

We are looking to recruit a motivated, committed andenthusiastic, Clinical Pharmacist with previous experience of working inGeneral practice to join our friendly general practice with a patientpopulation of around 11500 for 10 hours per week

Clinical staff includes two GP Partners, a team of locum GPs,two Practice Nurses and a Healthcare assistant.

Applicants must possess relevant qualifications, be flexibleand a team player.

Applicants should have completed 18 month CPPE Pathwaycourse and be an independent prescriber.

Main duties of the job

The role will involve working towards various qualityimprovement projects and to achieve PCN network DES.

The role will have a patient facing element either face toface or telephone - including chronic disease management, repeat prescriptionmanagement and structured medication reviews. This will also include themanagement and review of patients with polypharmacy.

The post holder will work within their clinical competenciesto provide specialist pharmaceutical support to all our patients and will beworking with members of the wider healthcare team to improve care and supportsafe and effective prescribing.

The post holder will need to be proactively involved inquality improvement and audit as well as managing aspects of the Quality andOutcomes Framework.

Bradford Student Health Service is a University practice located near the University of Bradford with a list size of over 11,000 patients. The majority of our patients are students aged 18-25 with a high number of overseas students.

We are a small friendly team that works collaboratively with the University of Bradford. We are a member of PCN 6 which is made up of 9 GP practices working together across the network area with ourcommunity partners to look after the health of our local population.

The post holder will work in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
