We are seeking Generic Support Workers to join either York and Shar (South Hambleton and Ryedale) Community Response Teams– to deliver high standards of multi-disciplinary interventions to patients within their own place of residence, to prevent avoidable admissions to acute care and to support discharges.

This role will be delivering interventions which support activities of daily living and promoting independence, as able.

Our well-established teams cover the historical city of York, as the rolling countryside within North Yorkshire. The team consists of Physiotherapists, Occupational Therapists, Nurses and Support Workers and works alongside Primary Care and Partners to provide Urgent Care Response, Frailty Hub as well as short term reablement/rehabilitation programmes.

You will undertake duties delegated by the Nursing, Physiotherapy and Occupational Therapy teams, and will be responsible for providing holistic, person centred compassionate health care to patient’s 18 years and over, supporting rehabilitation, reablement and recovery within the comfort of the patients home.

We provide a 7 day service, between the hours of 8am - 8pm; therefore successful candidates will take part in a rota that covers the hours of the service, however we do offer a flexible and varied shift pattern.

Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required
