Exciting opportunities have arisen for 2 Band 6 Care coordinators at our Southwark site. The successful candidate will hold a professional qualification of OT, Social Worker or RMN. Under the line management and clinical supervision from within the CMHT, the post holder will maintain and coordinate timely and appropriate interventions through holistic assessment for a caseload of service users of any age living in the community within the framework of CPA, and in line with the acute care pathway as outlined within the CMHT operational procedure

The successful candidate will Contribute to an effective communication process with appropriate and accurate information to patients, carers, family, friends, and colleagues. Engaging and listening carefully and actively and responding using appropriate language and accessible communication methods and skills. Developing communication skills to enable and support difficult conversations, acknowledging cultural differences and professional boundaries.
