MAIN RESPONSIBILITIES:


Professional / Ethical Practice

Being personally accountable for professional and ethical actions andensuring compliance with the NMC Professional Code of Conduct

Maintaining confidentiality, while communicating client information, in sucha way that preserves the dignity and privacy of the client and family/carers

Ensuring data is inputted onto local IT systems accurately and on time asrequired by the local employing organisation

Co-operating and assisting in research and survey programmes relating tothe client group or the nursing profession


Patient Care Delivery / Communication

Developing and maintaining effective communication networks with clients and other health professionals, statutory and voluntary agencies

Accurately and timely recording of all care given to the clients and reportany changes in the clients condition to the health visiting team, practiceteacher and the general practitioner or other member of the health / socialcare team as required

Attending and participate in staff meetings, case conferences and othermeetings as required

Identifying problems with communication systems and helping to improveits effectiveness with other professionals and team members

Ability to provide comprehensive and accurate written records that may beused in enquiries and / or court sessions

Ability to formally present and discuss with individuals and groups ideasand issues pertinent to Health Visiting and Public Health

Being responsible for the assessment of care needs for identified case load/ client group

Discussing and identifying who is responsible for the assessment ofidentified client groups

Identifying the multiple needs of the patient, family / carer, giving counseland advice appropriate to the situation

Using the knowledge and skills necessary to assess individuals and groupsin a holistic way taking into consideration of cultural differences

Promoting clients rights during the assessment process

Working within the primary care setting as a member of the health careteam, participating in activities, which address the health needs of thecaseload or client group

Providing and maintaining a high standard of care for clients in their homes,health centres, children centres and practice surgeries using a model ofcare, that is consistent with best evidence and within own scope of practiceand legislation

Agreeing in partnership with the individual client or group in the planning of,implementation of, and evaluation of a robust care plans

Ensuring the changing needs of individuals and groups are identified in atimely manner and ensure adjustments to programmes of care are made

Identify, plan and undertake specific health promotional activities withidentified target groups

Promoting and maintaining optimum health in clients / carers

Providing health education to all clients/ carers to include care and safetyof drugs, equipment and prevention of accidents

Giving advice on the health needs of the community

Actively involving the target group in the planning and implementation ofany proposed programme of intervention

Following local employing organisations policies and guidelines in allsuspected forms of abuse

Ensuring that concerns and identified potential risks are referred to theappropriate Medical Practitioner and other related agencies immediately.

The student will be expected to take responsibility for their adult learningand to inform their Practice Teacher and University contact should there beany exceptional circumstances preventing the completion of any aspects ofthe course.

It is stressed that some of the information, which is handled by the studenthealth visitors is of a highly confidential nature and should not be divulgedto another party nor should it be discussed with colleagues unless it isessential to do so.

The student will be expected to follow local organisations sickness andabsence policy, reporting sickness or absences to both the employer anduniversity.

The Specialist Community Public Health Nursing (SCPHN) (Health Visiting) course is offered full-time at Post Graduate Diploma level (Level 7), in partnership with Birmingham City University.

To engage in training in order to complete the BSc (Hons) Degree / PostgraduateDiploma in Specialist Community Public Health (Specialist Practitioner award inHealth Visiting).

This will involve liaising with all relevant parties within the practice area and at the assigned University in order to gain the desired 50% theory and 50% practical element of the course. In order to attain successful completion, the student will be required to work as part of a multi -disciplinary team within the primary care setting, carrying out clinical duties with clients as agreed with their Practice Teacher. The student will be accountable to the respective local manager according to localmanagement structures, and responsible to the allocated Practice Teacher for the term of training.
