This is anexciting time for PC24 as we are transforming and implementing new models ofcare across our services and increasing our input into providing support tointermediate care facilities.
This clinical role will requirethe postholder to have extensive experience in assessing the needs of olderpeople, including being able to work independently within the nursing andresidential care home environment.
Main duties of the job
The postholder will be required toundertake allocated review of patients via ward rounds using advancedcommunication skills to discuss and complete proactive individualised clinicalmanagement plans, with the aim being to reduce unnecessary hospital re-admissions.
In addition, they must have excellentinterpersonal skills to promote positiveworking relationships with care home staff, communitynursing teams, mental health services, palliative care teams and socialservices, to enable residents to meet their health goalsand be cared for in their preferred place of care.
