We are looking for an enthusiastic and capable nurse with relevant clinical and leadership experience to manage our Complex Care Team in Aberdeen City, whilst working closely with multi-professional colleagues and families to improve the pathway for young adults transitioning from children’s services. Applications from NMC registered nurses for Adults, Children and Young People and Learning Disability will be considered.
You will provide professional leadership and line management to a team of nurses and healthcare support workers who directly care for individuals with life limiting/life threatening physical conditions, which may be combined with a learning disability.
The role also involves expert advice and support for primary and secondary healthcare teams, families, guardians, social work and social care providers to better understand and meet the needs of this group of individuals. The role will support complex transitions to adult services, in addition to ongoing care.
The post is a new addition to our Community Nursing Team, and you will be supported by a Deputy Chief Nurse, as well as by your team leader colleagues across the range of Community Nursing services.

Hours predominantly Monday-Friday, however the team works 24/7 so applicants should be flexible and available to work at other times as required.
