The Personalised Care, Information and Support Manager for Cancer plays a vital role in ensuring care provided to patients with cancer meets their individual needs. You'll be joining a team who are dedicated to quality care for all patients. You'll oversee a programme of work which supports the cancer speciality teams in UHBW (Bristol & Weston sites) to deliver our organisations Personalised Care and Support (PCS) strategy.

Personalised cancer care means providing patients with access to care and support that meets their individual needs, from when they receive their cancer diagnosis, so that they can live as full, healthy and active a life as possible. This can include helping people to access financial, emotional, psychological and social support as well as providing them with information on managing their lifestyle, treatments and therapies available.

The NHS Long Term Plan for Cancer states that "where appropriate, every person diagnosed with cancer will have access to personalised care, including needs assessment, a care plan and health and wellbeing information and support."

UHBW is working towards offering these personalised care interventions to all people with a cancer diagnosis:

  • Personalised Care and Support Planning
  • End of Treatment Summaries
  • Primary Care Cancer Care Review
  • Health and Wellbeing Information and Support

  • Supporting the alignment of the delivery of Cancer PCS and CIS programmes, projects and initiatives to the strategic objectives of the Trust
  • Building capability across the organisation by coaching and training cancer services colleagues across UHBW in Cancer PCS and CIS service improvement and transformation methodologies (i.e. project management, Lean, service redesign, etc).
  • Involving patients, users, carers and members appropriately in shaping the PCS and CIS programmes, where appropriate.
  • Responsible for leading on the delivery of the specified Cancer PCSprogramme (including implementation of Holistic Needs Assessments, Health and Wellbeing events, Treatment Summaries and Risk Stratified Pathways).
  • Responsible for benefits realisation plan that demonstrates improved Cancer PCS performance in terms of quality, safety, people, cost and delivery.
  • Working closely with the UHBW Lead Cancer Nurse, senior clinical staff and senior management colleagues to plan, develop and deliver the PCS and CIS programmes that will improve patient care and patient experiences through enhanced quality of services
  • Line Management and professional support of Senior Cancer Support Managers in Bristol & Weston, and Macmillan Centre Managers.


Please note that this post is required to work between Bristol and Weston hospital sites across the week, with requirement to travel to community locations and regional locations at times.
