RINCIPAL DUTIES & AREAS OF RESPONSIBILITY
Clinical
- Will undertake specialist respiratory assessment of patients nursing needs, plan, implement and evaluate care delivery using clinical knowledge and clinical skills.
- Identify suitable patients for early supported discharge, plan and arrange discharge and provide individualised, holistic care plans to suit the patients needs.
- Review patients in the home environment following referral for admission avoidance. You will undertake advanced clinical assessment obtaining a history, clinical examination and conclude diagnosis implementing a treatment plan. You will also make the judgement if the patient can safely be managed at home or if they require hospital admission.
- Review patients in accident and emergency or ambulatory care unit for admission avoidance following an initial assessment by the medical team. If deemed suitable for home care you will plan and implement the patients discharge and arrange follow up at home.
- You will undertake frequent home visits to patients who are receiving an acute episode of PERT care ensuring their clinical condition is stable, maintaining a safe alternative to in -patient hospital care.
- You will ensure all documentation and communication is maintained to a high standard.
- You will liaise with members of the multi-disciplinary team and refer to other services and health care professionals where appropriate ensuring holistic care.
- Manage a patient caseload of those requiring long term non- invasive ventilation providing expert assessment, planning and evaluation.
- You will provide specialist respiratory nurse/respiratory education to patients on the wards undertaking in-patient asthma reviews, COPD management, inhaler optimisation and oxygen prescribing advice.
- You will perform in-patient oxygen assessments in accordance with best practice guidelines.
- You will routinely perform arterial blood gas analysis and other investigations such as Tosca monitoring within the patients own home.
- Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
- Participate in specialist outpatient clinics such as the home ventilation clinic and MDT MND clinic.
- Establish and maintain effective communication with patients and carers / relatives.
- Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.
- Recognise changes in patients condition that require the intervention of others and refer on as appropriate.
- When appropriate you will be expected to undertake a non-medical supplementary and independent prescribing qualification in order to prescribe within area of speciality for your patients.
- Work with the CNS / Team Leader to promote the development of nurses undertaking extended roles as considered appropriate.
Leadership
- Act as a role model providing specialist, clinical knowledge to clinical colleagues through MDT meetings for patients and carers / relatives.
- Undertake clinical supervision of nursing colleagues on an individual or group basis.
- Act as a resource for health care professionals by being visible, available, and accessible for support and advice in relation to the management of patients in the speciality.
- Participate in the development of services and assist the CNS Team Leader to implement change, linked to directorate business plans.
- Ensure the effective and efficient use of physical and financial resources.
- Participate in the development of strategies for delivering effective specialist care within a changing environment that improves the quality of care and health outcomes in the specialist area.
- Monitor health, safety and security of self and others and promote best practice in the clinical area.
- Participate in ensuring systems are in place for risk assessment and minimisation including promoting no blame cultures in order that near misses and incidents are reported.
- Participate in investigation of incidents/complaints when required,
- Act as an innovative, enthusiastic role model providing leadership, guidance and advice to staff on operational and professional issues promoting an open and honest and transparent culture.
Advocate
- Ensure that high standards of care are given and maintained and act when standards are not being met.
- Involve patients and carers in the planning and delivery of care.
- Act as the patients advocate and ambassador respecting patient confidentiality and privacy with respect for diverse cultural backgrounds and requirements.
- Support and enable patients to make informed choices.
- Recognise, prevent and diffuse potential areas of conflict and where required take a key role in the management of the outcome.
- Be proactive in the development of practice / services in the clinical area through the business planning process / service development and in conjunction with Ward Sister, Team Leader, Matron, General Manager, and Clinician.
Clinical Governance / Quality
- Ensure compliance with policies, procedures and clinical guidelines for self and others and participate in the development of the same.
- Continually monitor standards of care and contribute in the improvement of care through benchmarking, Nursing Charter Audits, Clinical Audit and Research.
- Support and facilitate research evidence-based practice in the specialist area.
- Disseminate good practice by pursuing opportunities to present work at national conferences or via publications.
- Promote patient and public involvement activities in the specialist area leading to service improvement.
- Promote peoples equality, diversity and rights.
- Identify clinical issues and incidents within the Trust that reduce the quality of care within the specialist service.
- Take part in activities to counter these to ensure the delivery of safe and effective care.
Education / Training
- Takes responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements.
- Participates in the supervision and the teaching of pre-post registration students, new staff members and junior medical staff.
- Provides education and training related to the speciality.
- Supports the Ward / Department Sister in ensuring the clinical environment is conducive to effective learning.

PERT Service
Due to the expansion of our highly regarded service, and the development of the Virtual Ward pathways and services, we have an exciting opportunity for a Respiratory Nurse/Physiotherapist Specialist to join the PERT Team.
We are looking to appoint highly a motivated, knowledgeable Respiratory Nurse/Physiotherapist who has a passion for delivering safe, high quality care to patients in the acute and community setting. The successful candidate would gain a wealth of knowledge and experience surrounding respiratory medicine. After a successful induction period you will be required to advise colleagues and patients in regards to acute and long term management of their condition. As part of the PERT team, you will be required to be self-motivated as the post requires some lone working, but as part of a supportive wider team.
We would consider a Band 5 Registered Nurse/physiotherapist, who has a genuine interest in respiratory medicine, as part of a development role. This post is suitable for a Band 5 who is willing and motivated to undertake further training to progress to a Band 6. Experience in managing patients with respiratory conditions would be an advantage but training will be provided for the right candidate.
Aims of the PERT service/main duties of the role:
- Admission avoidance service for patients who would usually require admission to hospital with an exacerbation of COPD, or other respiratory diagnosis.
