To provide day to day supervision and management of the Multi-Disciplinary Team Co-ordinator (MDTC) / Patient Pathway Coordinator(PPC) team. To work closely with clinicians, specialist nurses, secretaries and all members of the Multi-Disciplinary Teams (MDT) and associateddepartments, to ensure that patients on a cancer pathway are proactively and in a timely manner managed along their pathway. To co-ordinate the patient journey from referral, through diagnostics, treatment planning and treatment, highlighting any potential delaysin the pathway promptly. To ensure accurate data collection on Somerset in accordance with local and national requirements i.e. Cancer Waiting Times (CWT),Cancer Outcomes and Services Dataset (COSD, national cancer audits etc. To highlight relevant patients for discussion at the Multi-Disciplinary Meeting (MDT) and to ensure that the MDT coordinator (MDTC)receives all relevant information for the patients to be discussed. To provide absence cover for any member of the Patient Pathway Co-ordinator (PPC) or MDTC team as required. To maintain up to date knowledge of all cancer targets and processes relating to cancer pathway management and to work closely withthe Cancer Analyst to accurately collect data on Somerset Cancer Register for submission upload. To maintain awareness of Trust policies relating to the post and to fulfil mandatory training requirements as determined by the Trust.
