The post holder is responsible for the continuing management of the twilight patients and/or co-owners within their clinical setting, without supervision. This includes the assessment of careneeds, the development, implementation and evaluation of programs of care, the setting andmaintaining of standards and the supervision and deployment of the twilight team, if appropriate.The post holder will undertake the facilitation of educational programs for basic, post-basic and in-service training, in close liaison with the Learning and Development Team. To work closely with the Community Hub teams to facilitate patient-focused, innovative research-based practice, inline with Surrey Downs Health & Care objectives. To promote continuity of care between twilightand day services through effective communication and processes. To coordinate closely with allSurrey Downs Health & Care twilight teams to ensure a joined-up (seamless) service is deliveredand the teams are supported.
