Job responsibilities:

Assist in and perform routine tasks related to patient care in negotiation with other members of the nursing team and as directed by GPs.

General:

  • Work with other members of the nursing and healthcare team to organise and provide nursing services for the practice population
  • Provide nursing treatments to patients in participation with General Practitioners (GPs) or independently, following agreed protocols
  • Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to GPs as necessary
  • Advise patients on general healthcare and minor ailments with referral to GPs as necessary

Essential duties:

  • Cytology and sexual health screening
  • Wound care
  • Sexual health
  • Diabetes Management
  • Removal of sutures
  • Zoladex
  • Contraception
  • Routine immunisations/childhood immunisations

Non essential duties:

  • Travel health
  • COPD

  • Student/staff education and training

Participate in the education and training of students of all disciplines and other members of the nursing and healthcare assistant team.

Liaison:

Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality

Holmwood Corner Surgery are proud of our team which comprises 4 GP partners, seven salaried GPs, two pharmacists, FCP, a social prescriber, three practice nurses, a nursing associate and HCA supported by our experienced management team and administrative colleagues including a prescription clerk and a team streamlining workflow for clinicians. Our purpose-built premises serves a population of 13,900.

The surgery is looking for a friendly, professional, and efficient Practice Nurse to join our practice team. The right candidate will demonstrate strong interpersonal and communication skills for dealing with our diverse patient population and be able to work well in a close-knit team of both clinicians and non-clinicians. Good clinical knowledge and analytical skills are important to ensure successful consultations whilst working closely with the existing nursing team of three senior nurses, 1 Nursing Associate and 1 HCA.

Experience in long- term condition management including COPD, Asthma and Diabetes would be ideal as well as experience in contraception and family planning.

Previous experience in general practice is essential.

  • Deliver a high standard of patient care using high level autonomous clinical skills and a broad and in-depth theoretical knowledge base
  • Deal with presenting patients needs in a primary care setting. Responding effectively to patient need and ensuring patient choice and ease of access to services.
  • Promote healthy lifestyles and participate in national immunisation and screening programmes.
  • Enable patients to manage their long-term health care needs and to achieve their full potential in terms of independent living.
  • Carry out all duties in accordance with the NMC Code of professional conduct and with regard to the various local and national policies, guidelines and protocols.

  • Actively encourage and facilitate choices for patients by providing health promotion during routine screening procedures e.g.: new patient health checks, cervical cytology and long-term conditions management.
  • Communicate complex information to patients and relatives where needed with a variety of diverse needs, using negotiation and persuasive skills to motivate.
