Newbridge Surgery is a GPPractice with approximately 5,800 patients located near Tettenhall Village in Wolverhampton. We work as part of Wolverhampton Total Healthdelivering a Primary Care Network (PCN) service to approximately 58000 patientsacross Wolverhampton.

We currently have an opportunityfor an experienced Clinical Pharmacist to join the Surgery. Previous experience of working as a ClinicalPharmacist in a General Practice environment is essential.

The role is to improve health outcomes and the efficiency of the Primary Care team by providing direct, assessible and timely medicines expertise.To pro-actively transfer workload relating to medicines optimisation issues from other clinical staff to improve patient care and safety.

Main duties of the job

You will be working in a patient facing role with managing long term conditions; conducting medication reviews for patients in care homes and wider community. Other duties include; promoting self care, access and treat conditions for patients, medicines management and safer prescribing.

You will be an independent prescriber with demonstrable experience in an General Practice environment and knowledge of EMIS / Docman 10 is desirable.

Overseesystem for reconciliation of medicines from outpatient and discharge lettersand support safe transfer of care through liaison with hospital and communitycolleagues.

Liaisewith other stakeholders in the supply chain such as community pharmacists andappliance contractors to handle queries and manage supply problems.

Maintainand review a Medicines Handling Policy for the practice to ensure safeprocesses for self-administered medicines and controlled drugs.

Supportthe delivery of QOF, incentive schemes, QIPP and other quality or costseffectiveness initiatives.

Agreeand review Prescribing Formularies and Protocols and monitor compliance levels.

Improve the data quality of medicinesrecords and linking to conditions.
