We have a vacancy within our new Partnership Hub for a Patient Care Coordinator.
You will play a key role in delivering our patient experience strategy, ensuring our customers have a first-class experience and receive the care and support they need.
Role:
Patient Care Coordinator – 1-year fixed term contract, full time
Location:
Remote
Salary:
£27,300 per annum
Hours:
35 hours per week, working on a shift pattern, Monday to Friday between 8am and 6pm. We work on a rota basis between 8.00am and 4.00pm, 9.00am and 5.00pm and 10.00am and 6.00pm.
As a Patient Care Coordinator, you will:
- Facilitate the delivery of outstanding care by being responsible for the co-ordination of administrative tasks and processes, ensuring these are managed in accordance with targets and agreed timescales.
- Be responsible for providing a high standard of professional and patient focused services across a range of specialities and modalities.
- Ensure patient records are maintained and that all correspondence, reports and results are uploaded correctly and in a timely manner.
- Track patients in accordance with RTT and DMguidance and standards.
The ideal candidate will have:
- Excellent empathy, communication skills and exceptional attention to detail.
- Prior experience in a customer service role, ideally within a healthcare setting.
- Knowledge and understanding of RTT and DMreporting requirements.
- Ability to remain calm and professional in a very fast-paced, constantly evolving environment.
- Strong organisational skills and attention to detail.
- An ability to cope with the sensitive nature of work, and to deal with challenging and difficult behaviour in an appropriate manner.
