Key Tasks -
To deliver direct care, treatment and emotional support for patients as required within set protocols/policies in accordance with the patients individual care plan.
To contribute to the assessment, implementation and monitoring of patients and carers, identifying physical and/or psychological problems and liaising with the appropriate health and social care team members.
To work in collaboration with colleagues to ensure the delivery of high quality, safe and compassionate care and support.
To promote and maintain effective liaison with care team and other health and social agencies involved in the patients care.
To assist in the day to day organisation of the service of carrying out duties as delegated by the Care at Home Team Leader and Clinical Nurse Specialist.

The successful postholder will be required to work unsocial hours as required, including weekends/nights and bank holidays, delivering direct care, treatment and emotional support for patients as required within set protocols/policies in accordance with the patient's individual care plan.
They will work flexibly across all care services provided by Mountbatten, which will involve working in patients' own homes or other places of care, and work in collaboration with colleagues to ensure the delivery of high quality, safe and compassionate care and support.
This role requires promoting and maintaining effective liaison with the care team, and other health and social agencies involved in the patients' care.
