NHS@Home is an exciting model of care that bridges the gap between traditional hospital-based care by providing care in people’s home, supported by joint working with system partners and the use of appropriate remote clinical monitoring tools.
We are seeking a highly motivated, dynamic, and experienced Nurses to support the coordination and delivery of the NHS@Home service at UHBW.
The successful candidate will be responsible for coordinating the day-to-day delivery of the service, case management and supporting in-patient identification. The role will require collaborative working across the Bristol, North Somerset, and South Gloucestershire NHS@Home teams.

You will work closely with the senior NHS@Home team along with other collaborative partners to ensure the service operates efficiently and effectively, ensuring that high standards of patient care are always maintained.
This role is based out of our Weston General Hospital hub but may be required to work trustwide.

The NHS@Home Band 6 nurse will:
- Coordinate the daily operations of the NHS@Home service.
- Maintain an oversight of all patients being supported under the service.
- Support staff members undertaking patient visits and make autonomous decisions regarding patient management plans.
- Undertake accurate patient assessments within in-patient and out-patient settings.
- Support in-patient identification and manage referrals.
- Work with system partners to oversee system-wide service capacity.
- Assess patients and make comprehensive reports to the relevant MDTs.
- Support the Clinical Operational Lead and B7 with the development, implementation, and delivery of new clinical pathways to support service growth.
- Work collaboratively with Sirona and NBT staff organising NHS@Home caseload and managing referrals.
- Escalate clinical concerns appropriately.
- To work autonomously making clinically effective and appropriate decisions.
- Undertake community visits to deliver clinical care. Full UK driving license, appropriate car insurance and access to a car is essential
