Greater Manchester Mental Health is seeking an experienced Social Worker to join their Community Mental Health Team (CMHT) later life team at Etrop Court in Hemel Hempstead. The role involves providing a person-centered, statutory social work service for a caseload of patients, where you will conduct assessments to determine individual needs, develop care plans, and collaborate with other professionals to ensure effective support and safeguarding for service users.

Key responsibilities include offering counselling and support to clients and their families, advocating for clients' rights, coordinating services with external agencies, and maintaining accurate records of client interactions and progress.
