In line with the NHS Long Term Plan, Hertfordshire Community NHS Trust is providing clinical pharmacy services to PCNs in the local area. This is a unique opportunity for pharmacists to experience work in the rapidly expanding area of primary care, whilst retaining the support, benefits and familiarity of employment in an NHS organisation.
The Senior Pharmacist will provide expert input to the GP surgeries in a PCN, the patients they cover and locality services to achieve improvement in the quality and cost effectiveness of prescribing and achieve local and national health initiatives.
Utilising their prescribing qualification, the role will support the medication needs of the patients as well as educate other healthcare professionals on prescribing and the effective use of medicines.
The Senior Pharmacist will undertake clinical medication reviews and patient facing clinics, working in partnership with the patient's GP practice, nurses, care homes, community pharmacists, secondary care and the local authority. The successful applicant will need to be self-motivated and able to work on their own, whilst also integrating into a well-established team.
