We are looking for enthusiastic Advanced Clinical Practitioners (ACPs) to join our newly implemented Acute Medical Hospital at Home, Virtual Ward (VW). As an experienced ACP, you will be at the forefront of healthcare innovation,utilisingyour advanced clinical skills to provide exceptional, patient-centredcare in a VW environment. Your responsibilities will include conducting comprehensive face-to-face/ telephone assessments,devisingand implementingindividualisedcare plans, and collaborating with healthcare professionals across various disciplines. The Virtual Ward allows patients to receive the care they need at home (including care homes) safely, for a short period of time. The post holder will be expected to work as part of an MDT approach, as well as independently; providing clinical assessment, diagnostic decision making, and deliver treatment to patient's with urgent care needs in the community. The postholder will need to have the ability to priorities and triage the needs ofpatient's which may involve complex decision making, as well as considering referrals for further investigations where necessary, in line with local and national guidelines. The postholder will need to be able to recognise their limitations, ensure their clinical practice is safe and effective, and escalate appropriately to the dedicated team of senior medical colleagues asappropriate.
The postholder will be supported by the senior clinical team which includes consultants, trust doctors, lead advanced clinical practitioner, and senior pharmacists. There will be a daily board round and MDT meetings to discuss the patient's care needs and workload planning.
The postholder will be expected to regularly work weekends, bank holidays, and ideally be flexible to work shift patterns covering the hours of 08:00 - 20:00. All travel time will be included in your working day, and you will be eligible to claim mileage expenses. It is essential for the postholder to hold a full driving license for this role.
