Job summaryAn exciting opportunity has arisen to work within Trust Services to assist the Trust and Divisions with patient pathway management, ensuring patients are on the correct pathway and receive good communication about their care.
The role is predominantly home working.
Main duties of the jobWe are looking for a motivated team member to join our corporate Referral to Treatment team. You will assist divisions by contacting patients either by text message, email, letter and sometimes via phone to request an update on the patient's current status, ask if they still require an appointment and if they would be willing to travel to another hospital for their treatment if the wait time is shorter.
At times, the post holder will be required to proactively monitor and validate patients on clinical pathway, ensuring that access targets are met. This will involve managing groups of patients through the 'non-admitted' and 'admitted' element of their treatment pathway to enable treatment to be delivered within key access standards.
The successful candidate will possess the skills and key competencies to support the needs of the department, as stipulated in the person specification. You will be required to have excellent communication skills and have previous experience of communicating with patients. You will be required to be organised, diligent and have patient care as your primary concern. You should have an understanding of Referral to Treatment rules as well as awareness and understanding of other national targets. We will offer the successful candidate the relevant training, however previous experience of working with Careflow and Referral to Treatment pathways is preferred.
