An opportunity has arisen for a self-motivating and resourceful Rehabilitation Technician to join our busy Dementia & Delirium Service.
Based primarily within Northampton along with some community working as an Occupational Therapist/physiotherapist technician, you will be responsible for supporting the Band 6 Senior Occupational Therapist in their role of the assessment and treatment of Older Adult patients who are on the D&D pathway.
Your role will include motivating patients to achieve and maintain their optimum independence and assisting with the undertaking of various assessments, including mobility and activities of daily living. You will be expected to carry out plans of care as directed by the Senior staff member, as well as being able to work on your own initiative within your scope of professional practice.
The D&D service specialises in assisting with complex discharge planning for people with dementia and/or delirium who are acute hospital patients. The D&D model works closely with inpatient and community services in order to support patients and their families with the most appropriate discharge plan helping reduce delayed discharges and giving the patient the best chance of returning home.
The successful candidate must have excellent communication skills, be confident in their ability to support service users with their identified needs, and most importantly be a team player.
