We are looking to welcome new Continuing healthcare (CHC) coordinators to our assessment team. Previous CHC experience is beneficial but not essential. If you are a registered nurse, allied health professional or social worker and have the right skills, attitude and a desire to learn, we can support you to develop the specific skills required for Continuing Healthcare.
As a CHC coordinator you will be responsible for conducting comprehensive individual assessments of patients for both Funded Nursing Care, Fast Track and NHS Continuing Healthcare, case managing clients to ensure that their needs are effectively met and promoting Personal Health Budgets and personalisation to empower people with complex health needs live their best lives. You will be passionate about patient care and improving life opportunities.
To undertake continuing healthcare (checklists, decision support tool, fast track) assessments of individuals in a variety of settings (e.g., hospital, care home, individual's own home or via MS Teams/Zoom) ensuring the National framework is applied.
As part of the county wide team work flexibly to respond to the demands of the service to support the continuing healthcare process. You will assist in the implementation and review of personal health budgets and participate, as required, in safeguarding, best interest and adult protection meetings.
The ability to travel independently to visit patients at various locations across Cornwall is essential.
