About Us


An exciting opportunity exists to join our team the Vale Community Resource Service. The successful candidate would have the opportunity of working within Adult Services. You would support the social work function of the multidisciplinary integrated health and social care reablement team and at times the Integrated Discharge Service.

Vale Community Resource Service is an integrated Health, Social Care and Third Sector service based at Barry Hospital. We provide short term support and/or therapy to people living in their own home when they are first discharged from hospital or to prevent a hospital admission. Our aim is to maximise a person’s independence.

The team includes: Occupational Therapists, Social Workers, Physiotherapists, Speech and Language Therapists, Dieticians, Nurses, Reablement Coordinator staff and Reablement Support Workers, Pharmacy Technicians and Age Connect.

The Integrated Discharge Service work closely with Health and Age Connects to provide discharge planning support to citizens of the Vale of Glamorgan whilst they are in hospital. The Team is based at University Hospital Llandough (UHL) and consists of Social Workers and Accommodation Discharge Co-ordinators who work in partnership with health professionals to assist in planning and arranging the safe discharge of adults from the Princess of Wales Hospital, University Hospital of Wales, St David’s Hospital Cardiff, University Hospital Llandough and Barry Hospital.


About The Role


Pay Details: Grade 6:SCP 6.14 (£27,334pa) and 6.19 (£29,777pa) dependent on experience

Hours of Work / Working Pattern: Full Time -37 Hours (Hybrid)

Monday-Thursday 8.30am-5pm and Friday 8.30am-4.30pm

Permanent employment

Main Place of Work: A blended working arrangement which provides an opportunity to work from home and office. The expectation is that you would work across both Vale Community Resource Service and at times the Integrated Discharge Service sites.


About You


You will need:

  • Experience of working in a team.
  • Good standard of written and verbal communication skills.
  • Ability to organise and prioritise tasks.
  • Negotiation skills.
  • Ability to work on an inter-agency basis.
  • Good standard of written and verbal communication skills.
  • Ability to organise and prioritise tasks.

  • Ability to work on an inter-agency basis.
  • Understand the needs of service user group.
  • Experience of working with people in a paid, caring role or voluntary capacity
  • Some knowledge/understanding of ageing/illness/disability and its effects of individuals/families and their carers.
  • Ability to drive/travel throughout the Vale or between locations as appropriate.
  • Willingness to be supervised.
  • Willingness to change work location as necessary.
  • Full clean driving license
  • Knowledge of and commitment to the concepts outlined in the Council’s Equal Opportunities Policy.
  • Good standard of general education.
