This position is full-time with working hours of Monday to Friday 9-5.

The successful candidate will need to be able to demonstrate knowledge, skills and experience of person-centred assessments and working autonomously and collaboratively within a multi-agency environment. The candidate will need to work in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) including the promotion and review of Personal Health Budgets.

Main duties of the job

The successful candidate will need to be able to demonstrate knowledge, skills and experience of person-centred assessments. The role will involve working within a multi-agency environment to support the assessment and review process for NHS Continuing Healthcare (CHC) NHS Funded Nursing Care (FNC) including the promotion and review of Personal Health Budgets.

Experience undertaking mental capacity and best interests assessments is advantageous and an understanding of deprivation of liberty safeguards and the Court of Protection would be beneficial to the role. You will also need to be knowledgeable in report writing to ensure you can confidently complete a health needs assessment, personal care and support plan and the CHC Decision Support Tool.

You will need to be competent in using Microsoft Word, Excel and Outlook and also be confident in learning new systems for recording and report writing.

The successful applicant will be required to work both autonomously and collaboratively in a team and be able to adapt to a rapidly changing environment. About us

The North Nottinghamshire CHC delivery team supports the facilitation of CHC services in Mansfield, Ashfield, Newark and Sherwood. Staff working within the ICB are expected to have strong links with internal and external teams across this patch as well as the wider ICS. All individuals will have an identified base, but travel for assessments is expected as part of the role. Currently, the ICB are operating a hybrid working model allowing a mix of office and home working to support colleagues to achieve a better work/life balance.
