You will provide practical and emotional support (excluding personal care) to prevent avoidable readmissions and admissions, and enable our older people to recover safely at home.


Fixed Term Contract (5 days p/w on a 7-day rota) – to 31 March 2025 (extension subject to funding).

 is looking to recruit 
Service Co-ordinators 
to join our flagship Home from Hospital Service.

The service operates 9am to 5pm, seven days per week, 365 days a year supporting older people for up to six weeks post discharge from hospital.


You will provide practical and emotional support (excluding personal care) to prevent avoidable readmissions and admissions, and enable our older people to recover safely at home. Offering a helping hand and moral support when they need it most.


We are looking for resourceful, practical and empathetic individuals to deliver a high-quality service, and be part of a caring team. Previous experience in a caring occupation would be an advantage, but is not essential; however, compassion, dedication and commitment are.
