We are seeking an enthusiastic and motivated nurse to join our well-established Community Intellectual Disability Team's.
As a Community Learning Disability Nurse within the CIDT you will provide support and advice around the mental and physical health needs of people living within the community who's needs cannot be met by mainstream services.
The IDD service provides specialist care for those aged over 18 with intellectual disabilities and focusing on those whose needs cannot be met by a mainstream provision. We work to improve the mental and physical wellbeing of our service users, providing assessment, treatment, advice and support on less complex needs, specialist time-limited care to those with more complex difficulties, and intensive intervention and care coordination to those with the most severe and complex needs.
Within the IDD Service, we take a professional approach to meeting individual needs and ensuring that people receive the support they require to live a fulfilling life within their local community. Assessment and treatment is delivered by our Community Learning Disability Teams across Nottinghamshire.
There is one post in the South of the County covering Broxtowe, Gedling and Rushcliffe and another in the North, covering Newark and Bassetlaw and the surrounding areas.
The successful post holder will provide a defined level of specialist assessment and access to interventions for a diverse patient group and their families under the care programme approach, who present with significant Intellectual disabilities disorders.
Provide realistic timely or occasionally intensive support to patients within their own community setting.
Demonstrate active clinical and managerial leadership skills within the team, providing education and supervision to junior staff members and other professionals.
