The All Age Disability Service is a social care staff group that transferred to the NHS as part of the Health and Social Care Integration Agenda.
Are you a qualified social worker with at least two years experience looking for a new opportunity? We have a vacancy working with children within our Children with Disabilities Service.
Our team works with children with severe learning disabilities to support them, and their carers, to achieve their outcomes. CWP is at the forefront of health and social care integration and this service lends itself to developing new, innovative ways of working together.
Within the CWD Team you will be undertaking assessments, support plans and reviews, working with children with learning disabilities, their carers and providers. You will be a person centred practitioner and will complete support plans, mental capacity assessments, be involved in court work and safeguarding enquiries.
At least two years experience of working as a social worker is desirable. Experience of working with children with complex needs is also desirable but what is crucial is a genuine wish to work with children with learning disabilities to meet their own goals and aspirations.
An understanding of asset and strength-based approaches to assessment and support planning is vital. In return we can offer you a clear career progression framework with a range of training opportunities.
Commensurate with demonstrable level of practice and continued professional development, work within the Standards of conduct, performance and ethics as described by Social Work England ensuring compliance with legal, organisational and multi-agency requirements and working independently, carry a caseload and undertake a range of Child and Family, and safeguarding assessments of complex or unusual cases including appropriate management of risk.
Ensure the safeguarding and duty of care of children with learning disabilities, families and carers in ways that balance promotion of well-being, independence choice and control with the need to protect people from abuse and neglect.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
Our aim is to help people to be the best they can be, adopting a compassionate, person-centred approach to everything we do. We were recently rated as Outstanding for Caring by the Care Quality Commission with a Good rating overall.
We offer a variety of roles at all levels, so whether you are just starting out in your career or looking to use your skills and experience in a new role, CWP is the right employer for you.
CWP are committed to making our recruitment practices as inclusive as possible for everyone. We are committed to promoting equality and diversity and developing a culture that values differences.
CWP is proud to be an accredited Veteran Aware organisation helping to provide and share best practice in NHS care for our armed forces community. As a veteran aware organisation, CWP is committed to delivering on the principles of the Armed Force Covenant and has a ‘Bronze’ award with the MOD Defence Employer Recognition Scheme (DERS).
The trust offers a Guaranteed Interview Scheme to any armed forces community applicants who meet the essential criteria for the post and encourages applications from armed forces reservists or cadet force adult volunteers.
We welcome applications from people who have direct experience of accessing our services.
