Job summaryThe Urgent Community Response Service provides nursing, therapy and reablement to adults to support them to remain in their home avoiding attendance and admission to hospital and to return home following a hospital stay.
Operating 365 days per year, the service receives referrals from a range of health and care professionals and triages, assesses, and delivers care to the patient over several days to ensure the patient can remain safely at home.
The postholder will be required to work autonomously as an Advanced Clinical Practitioner, using these skills to assess, investigate, diagnose, treat and manage patients who may have co-existing and diverse factors making management complex. They will develop, utilise and maintain skills beyond the scope of normal practice (for example advanced physical assessment, diagnostics and prescribing). Within the role there will be a requirement to plan and direct the management of patients including accessing services within the multi-disciplinary team and wider health, social care, education, vocational and voluntary sector services as appropriate to the role. The postholder will lead on the strategic development of Trust-wide services in their specialist area, working with other strategic and clinical leads as appropriate. They will also lead and participate in education programmes designed to support other clinical staff skills within their specialty.
Main duties of the jobThe post holder will be required to communicate complex/sensitive information to patients, service users and their families and carers where there might be barriers to understanding.
