This role would suit someone with experience and/or knowledge of clinical nutrition as well as a desire to improve the nutrition of the local population and support vulnerable patient groups.
An opportunity has arisen to work as a Dietetic Assistant Practitioner based in Bolton to work alongside our clinical dietitians. You will be well supported by our enthusiastic and friendly community team delivering Nutrition and Dietetic services across Bolton.
The 0.8 WTE (30 hours) role involves working alongside registered dietitians to provide tailored nutrition support advice to patients, first line advice to follow-up patients for type 2 Diabetes and weight management patients, day to day administration and problem solving to improve department processes.
You will have an allocated caseload. The work will be a combination of computer work, face to face in care home, patient’s homes and outpatient clinics or telephone contacts and interaction with other professionals.
The post holder will ensure nutritional care plans agreed by the dietitian and patient are implemented with monitoring of outcomes. Across a number of clinical conditions.
