We currently have an exciting opportunity for a
Hospital Discharge Housing Support Worker
to join our Sycamore Service in
Bridgend,
providing housing related support in a psychologically informed environment to older people within the locality.
This is a permanent role working 36 hours per week, with a pay rate of £12.55 per hour.
What You'll Do
This role is an opportunity for a positive and proactive individual to build key partnerships to support individuals through the hospital discharge process and back into their own homes in the community safely and to reduce bed-blocking.
What You'll Need
To be successful in this role you'll have experience of supporting individuals with varying complex support needs. You'll need excellent interpersonal skills with the ability to relate to and build supportive relationships. Experience of working in working in a psychologically/ trauma informed way, with experience of supporting people affected by ACE's or supporting people with mental health issues would be advantageous within this role.
A qualification in Social Care or related discipline is desirable but not essential.
You will need a full, current driving licence and use of a vehicle.
