Hospital at HomeTeam Coordinator – Band 4


An Opportunity have arisen for a team coordinator within the Aberdeen City HSCP Hospital @ Home (H@H) team based on Woodend Hospital site. If you are a suitably motivated team player who can communicate across a range of disciplines we would love to hear from you.

Hospital at Home provides intensive, hospital-level care for acute conditions that would normally require an acute hospital bed, in a patient's home for a short episode through our multidisciplinary healthcare team. We continue to grow a substantive workforce and prioritise the wellbeing and safety of our staff, patients and their families.

This exciting new team coordinator role will support the Team Leader and wider management team to ensure the team work effectively, safely and efficiently and receive the cohesive support required to work within a high pressure environment, dedicated to delivering a safe, high quality service in the patient’s own home.

Full and part time applications would be welcomed. Part time applicants should include their preferred working hours in their application. The service is operational between over 7 days per week.

The ability and willingness to travel throughout the Aberdeen City area is essential for this post. The successful candidate must therefore hold a full UK driving licence.

This post is available on a fixed term/secondment basis for 12 months. If you are currently employed by the NHS on a permanent contract, you must have a discussion with your manager and have a secondment agreed in principal before submitting your application. Please include this within your application.
