Are you available towork Friday 6.30pm-11pm, Saturday 10am-10pm and Sunday 8am-8pm? Primary Care 24 (PC24) are excited to sharethe position of Urgent Care Clinician (UCC) on a part time,permanent basis.
Asa RegisteredNurse or Paramedic Practitioner, you will have extensiveexperience in emergency health care settings, significant expertise in clinicaltriage and the ability to assess the needs of patients referred for care whohave reached a range of dispositions from NHS111, to the Out of Hours service.
Main duties of the job- Develop and sustain productive workingrelationships with colleagues to provide clinical support to Clinical Advisorsand Call Centre Managers on shift, ensuring safe effective working.
- Reviewpatients awaiting contact to provide oversight and minimise clinical risk byactively filtering for potential signposting to more appropriate services.
- Basedaround a Red Flag approach of triage, prioritise patients in line with theservice escalation plans which include the elderly, palliative patients andthose who have a risk of deterioration.
- Applyclinical skills and reasoning to ensure patients and transferred safely betweendifferent clinical dispositions and signposting when required and safe to doso.
- Lead shift safety huddles to provideupdates and information to clinical teams. Acting assingle point of contact and oversight for referrals and co-ordination ofpatients awaiting contact
- Liaise with nominatedmedical/nursing leadership team to provide oversight and co-ordination ofpatients awaiting contact.
- Undertake robust clinicaltriage and allocate to appropriate clinical teams based on available skill mix.
- Identify high risk groups or patientswho may need escalation based on their clinical priority.
