The Confederation, Hillingdon CIC is seeking an experienced Physiotherapist for the role of PCN First Contact Physiotherapist in Uxbridge, specifically for GP Practices within the Celandine & Metrocare PCN in Ruislip. This Band 7 position involves working within a multidisciplinary team, including Clinical Pharmacists and Social Prescribing Link Workers, to provide direct patient care by assessing and treating musculoskeletal conditions. The role focuses on initial assessments, treatment planning, and ongoing care, with an emphasis on frailty and fall prevention. Candidates will also have the opportunity to enhance professional skills through peer learning and support from a named GP Supervisor.

In this clinical role, the First Contact Physiotherapist will act as a primary point of care, making diagnostic decisions independently of GP referrals. Responsibilities include conducting comprehensive assessments, interpreting diagnostic results, and developing management plans. The physiotherapist will promote self-care, encourage behavioral changes for optimal health outcomes, and facilitate improved physical activity and independence among patients while working collaboratively across community, acute, and voluntary sectors.
