This is an incredible opportunity for a registered children's nurse and/or primary care nurse who are keen to develop into strategic roles and contribute to the transformation of services and make a difference at a system wide level.

This role will focus on those children and young people with respiratory disease, predominantly although not exclusively asthma. Asthma is the most common long term condition effecting children and young people in the UK and one of the most common reasons for children and young people to attend hospital in an emergency. The role will also contribute to the development and transformation of children and young people accessing urgent and emergency care services / urgent treatments centres and/or primary care settings, who provide clinical advice to children and young people with minor illnesses.

As Children & Young People's Respiratory and Urgent and Emergency care nurse specialist you will work as part of a supportive and dynamic children and young people's team, within the quality and nursing directorate of Humber and North Yorkshire Integrated Care Board. The post holder will support the Respiratory and UEC Lead to deliver the mandate set out in the NHY CYP delivery plan, with a specific focus on respiratory disease and Urgent and Emergency Care across the HNY geography.

The post holder will act as a champion for CYP respiratory and urgent and emergency care and provide clinical advice and leadership in these respects:

They will support and provide professional leadership to the HNY system partnerships and support the development of a system wide strategy and implementation plan in relation to respiratory disease and urgent & emergency care in CYP.

They will be expected to develop key relationships across the system partnership including primary and secondary care providers and ensure robust clinical governance is embedded within all work projects. This includes ensuring a commitment to high quality evidenced based practice.

They will help to facilitate improvements in patient pathways, policies, clinical standards, and quality of care.

They will develop a business intelligence and population health methodology and subsequently lead work projects in relation to respiratory disease and UEC.

They will provide clinical advice and leadership in these respects:

They will help to ensure health colleagues treating CYP with respiratory disease and UEC needs have access to the appropriate level of training, contributing to the delivery of training and education. There will be a key focus on asthma and they will work closely with the ICS Respiratory & UEC lead to deliver the commitments set out in the National Bundle of Care for CYP with Asthma.

They will work closely with the ICS Clinical Lead for asthma to deliver the commitments set out in the National Bundle of Care for CYP with Asthma.

They will take a lead role in the implementation of a risk stratification process to identify those CYP most at risk of asthma exacerbations. This includes providing specialist advice and support to PCN groups.

Another key element of the role will be to embed the asthma friendly schools imitative across the ICS geography and be responsible for registration and monitoring of schools.

Develop and lead networks in relation to Asthma Friendly schools within the 6 local authority/ place footprints The postholder will be allocated a work base within the ICS geography, although a hybrid flexible working arrangement can be facilitated as long as it meets the service needs. There will be strong links to primary and secondary care and will work collaboratively with all members of the multidisciplinary team including general practitioners, practice nurses, paediatricians, and school nurses to deliver improvements in consistency and quality of care in the community. They will help to decrease outpatient activity, emergency department activity and primary care activity in relation to asthma through better control and access to more specialist input in the community. This will include regular face to face contacts with patients and professionals across multiple settings across the ICS and travelling across the geography will be expected. Strategic work in relation to UEC strategies will include systemwide consultation and collaboration with all partners in developing new models/ framework in relation to reducing demand on paediatric UEC Services. This will include reviewing of existing good practice. locally and nationally a sound knowledge of research and evidence based practice, including risk and impact assessments when managing the change process. Other crucial partners will be the ICB commissioning team, and this role will include leading and informing partners in the development of new service specifications and Key performance indicators in relation to delivery and impact. The role will also include working with the HNY Healthier together team and promoting the website at a variety of venues which will include other multi agency professionals and the VCSE. It will also include face to face promotion activities with community groups. The ICS covers a large geographical footprint including Northeast Lincolnshire, North Lincolnshire, Hull, East Riding, York & North Yorkshire. Efficient diary planning and working with other colleagues across the system will be required to make the best use of time resources.

The post holder will also be required to work across the different work streams within the CYP team and where appropriate collaborate and provide support. For example working with the HNY Healthier Together team and promoting the website at a variety of venues, to a range of multiangencies partners and to community groups.

You will be expected to work independently as well as part of the team and have a leadership role in driving change. This is likely to include organising and attending meetings sometimes chairing, facilitating training sessions and developing new relationships across organisational boundaries e.g. education services. You must be able to demonstrate confidence and clarity in a variety of situations and be able to build positive relationships across the system to support effective delivery.

There is a flexible working policy in place and there is an expectation to work from a core base on a weekly basis. You will also be required to work across the ICS geography dependent on needs, this will include provider settings and travel will be paid from your core base. Team meetings are held at West Offices in York and supervision to be confirmed with line manager on appointment. This is just opposite the mainline train station in York so very commutable.
