Central and West Warrington PCN 25/10/2024 FAMILY UNIFORM CASELOAD PHARMACY ROLE MODEL

**INTERVIEWS WILL BE HELD WEEK COMMENCING 26TH NOVEMBER** The postholder will join our existing team of 8 Pharmacists and 3 Pharmacy Technicians to support the 7 GP Practices of CWW PCN, as per rota scheduling to manage the caseloads of each Practice and delivering the defined requirements to meet the Primary Care Network DES and IIF. This post will be managed by a Lead Practice, with sessions and clinical work taking place across the allocated Practices(s) weekly, as per rotas. The postholder will be re
