Hayling Island & Emsworth PCN 22/10/2024 HOLISTIC MENTAL HEALTH MULTI-DISCIPLINARY

We are looking for a committed and compassionate Social Prescriber to join our multi-disciplinary primary care team. You will be part of our Primary Care Network Team, working across 3 GP Practices on Hayling Island and Emsworth, Hampshire. You will be required to work across our 3 GP Practices. Social prescribing empowers people to take control of their health and wellbeing, taking an holistic approach and connecting people to community groups and statutory services for practical and emotional support.
