Rehabilitation Support Worker - Band 3 Chippenham Community Team Full and Part time available - Permanent We are unable to offer overseas sponsorship currently for this role. Chippenham Community Team have an exciting vacancy for a Rehab Support Worker. The role of a Rehab Support worker is to work closely with a wider team to support individuals in their own homes for a short period after discharge from hospital. We provide support and rehabilitation to promote independence in activities of daily living.
