Job Title: Advanced Nurse Practitioner (ANP) Location: Langdon Hills Medical Centre, SS16 6SA Salary: Competitive, based on experience Hours: Part-time 18 hours across 2 days
We are seeking a highly skilled and motivated Advanced Nurse Practitioner (ANP) to join our friendly and well-established team at Langdon Hills Medical Centre.
This role offers an exciting opportunity to work as part of a dynamic, supportive environment, focused on delivering high-quality care to our local community. The successful candidate will play a key role in supporting our frail elderly population, as well as managing minor illness and chronic disease care. This position also forms part of our evolving model of care, delivered in collaboration with our Primary Care Network (PCN) and the Integrated Neighbourhood Team.

- Patient Assessment:Conducting thorough physical exams, taking medical histories, and ordering diagnostic tests.
- Diagnosis and Treatment:Making independent diagnoses, developing treatment plans, and prescribing medications within their scope.
- Chronic Disease Management:Managing chronic conditions like diabetes and COPD, working with patients to achieve optimal health.
- Acute Care:Providing care for acute presentations and minor health problems.
- Referrals:Making appropriate referrals to specialists or organizing emergency admissions when needed.
- Investigations:Ordering necessary blood tests, radiology, and other investigations.

We are seeking a highly skilled and compassionate Advanced Nurse Practitioner (ANP) to join our Primary Care Network (PCN) and provide dedicated support for care home residents. This role is essential in delivering proactive, patient-centered care, ensuring residents receive timely assessments, treatment, and ongoing management of their health conditions.
Key ResponsibilitiesConduct regular care home rounds, assessing residents' health needs.
Provide clinical assessments, diagnosis, and treatment plans.
Work collaboratively with GPs, pharmacists, and multidisciplinary teams.
Support medication reviews, ensuring safe prescribing practices.
Lead on preventative care, reducing unnecessary hospital admissions.
Offer guidance and training to care home staff on best practices.
Maintain accurate clinical documentation and contribute to quality improvement initiatives.
Registered Nurse (NMC) qualification with Advanced Practice training.
Independent Prescriber (V300) qualification (essential).
Experience in elderly care, long-term conditions, and complex case management.
Strong clinical decision-making and leadership skills.
Ability to work autonomously while collaborating within a team.
