The future is exciting. Are you ready to join Primary Care Sheffield (PCS) as one of our Advanced Nurse Practitioner growing collaboration within health and social care in Sheffield provides PCS with a wide range of opportunities which can improve patient pathways and reduce cost.
This post is for 14 hours 45 minutesper week.
The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the PSC practices to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required.
In order to work at this level NMC requirement for advanced practice must be met. To be considered for this exciting role you must have: a minor illness qualification and a Non Medical Prescribing qualification (V300)
The work to be undertaken, could be as part of a group wide triage team or within one of our seven practices.
- Comprehensively assess, review and evaluate the needs of patients with undifferentiated healthcare needs
- Undertake clinical assessment and providing the necessary treatment
- Request diagnostic tests only where clinically indicated in line with organisational, local and national guidelines and where appropriate
- Develop a plan of care in conjunction with the patient
- To utilise effective communication to aid onward referrals to specialists as appropriate
- To work in an autonomous manner without direct supervision whilst recognising limitation of the practitioner role
- To carry out enhanced clinical skills as required
- To keep accurate and immediate records of all interventions fully utilising software systems in place
- To encourage clients to carry out self-care as appropriate
- To maintain patient confidentiality

Clinical Responsibilities
Comprehensively assess, review and evaluate the needs of patients with undifferentiated healthcare needs to improve their physical, psychological and mental health wellbeing to promote self-care and reduce the need for hospitalisation. Including the needs of Children from the date of their first birthday.
Undertaking clinical assessment and providing the necessary treatment including the use of Independent Prescribing in line with local/national policies, procedures and guidelines. This may be face to face or via other remote communication methods (such as Telephone triage)
Request diagnostic tests only where clinically indicated in line with organisational, local and national guidelines and were appropriate.
Develop a plan of care in conjunction with the patient, with the application of clinical knowledge and, analysis of symptoms and data, identification of risk factors associated with those conditions, recognition of early signs of acute illness, involving the patients and the carers in the care plan and ensuring it is understood, documenting safety netting plans and processes
To utilise effective communication to aid onward referrals to specialists as appropriate. Facilitating a coordinated primary healthcare response to service users needs
To work in an autonomous manner without direct supervision whilst recognising limitation of the practitioner role
To carry out enhanced clinical skills as required.
To keep accurate and immediate records of all interventions fully utilising software systems in place (namely System one and C- The Signs).
To encourage clients to carry out self-care as appropriate through effective communication of health care using written information as appropriate
To maintain patient confidentiality as per Primary Care Sheffields Policies.
Professional
To work within the NMC Code of Professional Conduct or other relevant Codes of Conduct for Advance Nurse Practitioners.
To act in accordance with and promote PCSs values at all times
To act as an ambassador for PCS and build professional relationship with our primary care partners, voluntary organisations and other outside agencies
To be committed to working flexibly and respond positively to the changing needs of the service
To promote and facilitate evidence-based practice
To be committed to providing a high level of customer service and patient experience
Maintain professional awareness and attend appropriate training as necessary
To participate in the appraisal process and development of own Personal Development Plan to include clinical supervision and Revalidation
To develop a culture within PCS of continuing professional development, supporting nursing others to enhance their knowledge base and skill set.
To provide clinical support and mentorship to others including Nurses, Nurse associates, HCAs to meet the requirements of their own personal/professional development plans. This may include, where appropriate assisting with the mentoring nurses through work-based learning programmes.
To support/facilitate the provision of Clinical Supervision in accordance with organisational policy.
Continuous Personal Development
To maintain current professional registration and to ensure that all personal responsibilities in relation to this and to the revalidation process are met, including maintenance of a professional portfolio, evidence of practice hours, professional development planning and reflective writing.
To meet the organisational requirements for mandatory training appropriate to the role.
To recognise own professional limitations, practising accordingly, seeking advice and support as appropriate.
To take an active role in the organisational appraisal process and develop an appropriate and relevant personal development plan and work-based objectives.
To have awareness of the relevant organisational and professional Safeguarding policies and guidance and to act in accordance with their direction, attending update training as required.
The post holder will have regular 1:1s with line mangers to support ongoing development and review performance.
Management/Leadership
To participate in the training and induction of new members of the team
To participate in the mentorship, support and training of other professionals who may be on a training program.
Information will be communicated to a variety of individuals including health and social care professionals, patients, carers where there are potential barriers to communication.
Communication
Act as an integral part of the multidisciplinary Team
Disseminating information within the primary health care team
Communicate effectively with other professionals and managers, sometimes with different agendas and priorities.
Provide high quality written referrals, reports and any other written documentation as required.
To build professional relationship with primary care partners, voluntary organisations and other outside agencies
Physical Effort
A combination of sitting, standing and walking with little requirement for physical effort.
Able to fulfil physical demands of post, this may involve moving and handling.
Travel to any nominated site where the service is to be delivered
Carrying out diagnostic assessments
Mental Effort
Frequent concentration carrying out clinical assessments of patients presenting with undifferentiated conditions
Work flexibly in a wide range of setting including newly developing services.
Working under pressure at times
Working with complex, time consuming cases in difficult situations including mental health concerns.
Supporting others in distressing circumstances e.g. safeguarding referrals
Emotional Effort
May encounter distressing situations, possible evidence of neglect or abuse and will need to report these incidences,
Support given to other members of team in challenging situations
Advocacy of clients/carers
Highlighting of safeguarding cases and making referrals.
General
The post holder may be required to work within the boundaries of the professional abilities at any identified site where PCS services are delivered.
