Farnham Park Health Group is looking for an experienced and motivated Advanced Paramedic Practitioner (APP) to enhance their multidisciplinary team. The role primarily involves providing high-quality patient care for both acute and chronic conditions, conducting home visits, and engaging in Quality and Outcomes Framework (QOF) initiatives. Key responsibilities include reviewing and actioning blood test results, prescribing medications within competency, and supporting administrative functions to ensure effective patient management. The successful candidate will collaborate with GPs, nurses, and other healthcare professionals, ensuring a patient-centered approach while also assisting in managing care for housebound patients and those in care homes.
