d and motivated assistant practitioner with a keen interest in the holistic working model, to join the At home clinical team which is part of Urgent Community Response, Pathway 1 and Reablement services.The service consists of a multi-disciplinary team that includes Physiotherapists, Occupational Therapists, Nurses, Registered Mental health nurse, Assistant Practitioners, Rehabilitation Support Workers and administrative support. The aim of the MDT is to provide assessment and rehabilitation to patients who require urgent intervention to prevent hospital admission, support and assessment to facilitate hospital discharge and further reablement to patient who maybe at risk of deterioration or hospitalisation.We are a dynamic, developing organisation with well recognised rehabilitation services providing assessment, education, advice, support and rehabilitation for patients within our local community.
Main duties of the job
The successful candidates will be expected to work holistically alongside the multidisaplinary team which consists of Physiotherapist, occupational therapists, Nurses, Registered mental health nurses and rehabilitation support works. Our team provide comprehensive assessments to patients who are at risk of hospital admission, along with facilitating discharges from the Acute hospitals.You will need to be able to use your own initiative within your own limitations to support the clinicians to provide gold standard quality care to our patients within the local community.The ideal candidate would be someone with

of working within the health and social care sector, who can demonstrate they have the skills and qualities to work holistically in a fast paced environment. You will be expected to complete holistic assessments, order and assess equipment and minor adaptations within your scope of practice, assess, review and set goals for patients.In return you will be offered a comprehensive induction, access to internal and external training, exposure to a range of clinical expertise within our team, along with regular supervision.
