As an Assistant Practitioner with Royal Devon University Healthcare NHS Foundation Trust based in Cullompton (EX15), you will manage your own community rehabilitation caseload under the supervision of qualified Physiotherapists and Occupational Therapists. The role covers post-operative rehabilitation, long‑term condition management, falls prevention classes and same‑day equipment assessments to help patients remain safe and independent at home. This permanent, part‑time Band 4 post (salary £27,485–£30,162 pro rata) requires a Level 5 diploma, strong moving and handling skills, excellent communication, good IT ability to use MyCare, and a full driving licence with access to a vehicle. Shifts are a mix of early, late and weekend duties as part of a supportive rota, working closely with district nurses, social workers and the Urgent Community Response team to deliver joined‑up care.
