This role involves delivering compassionate end-of-life care, including pain relief and emotional support for patients and their families. You will also manage a range of clinical needs such as catheter care, wound management, medication administration, and urgent interventions for minor trauma.
Working closely with out-of-hours services including GPs, 111, and the ambulance service, you will respond to referrals and provide care in patients' homes. With the right training and support, youll build the confidence to work autonomously, backed by a skilled and supportive team.
This position is ideal for an experienced nurse looking to grow their skills in a community setting. We are committed to supporting your professional development and clinical autonomy.
The post holder will provide safe, high-quality nursing care in line with clinical protocols, working closely with community teams to support person-centred care. They will assist in the day-to-day leadership and coordination of the nursing team, undertaking delegated tasks from senior colleagues and escalating concerns appropriately. A focus on promoting patient autonomy and maintaining accurate, professional documentation is central to this role.
They will contribute to multidisciplinary care, support health promotion, and ensure evidence-based practice underpins all care delivery. The role includes upholding high standards, delivering compassionate, respectful care, and actively engaging in continuous learning and development.
Additional responsibilities involve supporting student learning, upholding equity and diversity, and addressing concerns or complaints in collaboration with the wider team. The post holder will also assist in the integration of digital tools, ensuring compliance with data protection and safeguarding standards.
Flexibility is essential, with shifts including evenings, weekends, and Bank Holidays. Access to suitable transport is required, and an Enhanced DBS check is a condition of employment.
