Job description
Central London Community Health Trust, nestled in vibrant Fulham, seeks a dedicated Bank Medicines Optimisation Pharmacist to deliver an exceptional clinical pharmacy service within our Community Hospitals. This role calls for a dynamic individual eager to support patient care transitions from secondary to primary settings, ensuring continuity and excellence in pharmaceutical care. As part of the Medicines Management team, you'll be vital in shaping a service that reflects national and local priorities. Your expertise will be pivotal across various settings, such as Rehabilitation Units and Palliative Care, supported by our Lead Bedded Services Pharmacist. Flexibility and mobility within London’s Bedded Services sites are necessary, facilitating the delivery of safe, cost-effective care in line with guidelines from NICE, NHSE/I, and PHE.
In this role, you'll spearhead individualised patient care, undertaking medicines reconciliation and comprehensive prescription screening. You'll provide guidance to Pharmacy Technicians and contribute to developing medicines-related policies, protocols, and Patient Group Directions. Participation in creating engaging content for the Medicines Management newsletter will showcase your expertise. Moreover, your involvement in implementing national and local prescribing strategies and supporting optimisation projects will be crucial.
