Support people in BD4 to quit smoking as the Breathe Co‑ordinator with Healthy Lifestyle Solutions CIC. Working from clinical records and alongside the Social Prescribing team, you will offer very brief interventions, signpost to the Living Well Swap to Stop offer or a three‑month free vape, and follow patients through group reviews. The role uses a trauma‑informed, person‑centred approach and draws on motivational interviewing, coaching and MECC to start quit attempts. You will help set up targeted group support for pregnant people and older adults with COPD, develop peer‑led groups, deliver outreach at community venues and schools, and take referrals from GP practices and a wide range of local agencies. Accurate record keeping, monitoring and a DBS check are required.

This fixed‑term, part‑time job‑share post (8 months) is based at Tong Medical Practice, BD4 9QA, serving four local GP practices and working closely with BD4+ PCN, Restore Recovery College and Bradford Council Living Well.
