Job description
Care Coordinator at Churchill Medical Centre (Kingston upon Thames) working across the Churchill, Canbury, Orchard and Berrylands PCN to support around 50,000 patients. This practical, people-focused role combines administration and care coordination: you will manage clinical correspondence and incoming email queries, process ANIMA triage requests, scan and file records, action hospital and community communications, support cancer care reviews and death protocols, handle SARs and screening invitations, and help promote vaccination and health education. You will organise appointments and follow-ups, liaise with multi‑agency teams, monitor tasks through regular audits and support delivery against QOF/KPIs. The post is permanent with full‑ or part‑time hours available, pays £12.21 per hour and requires a DBS check; GCSEs in English and maths (or equivalent) are essential, with NVQ Level 2 in Business Administration and experience in primary care or social care desirable. We’re looking for someone caring, calm under pressure, highly organised and adaptable — comfortable working independently and as part of a multidisciplinary team.
