Primary Care Network 5(PCN5) is looking for an enthusiastic,reliable care coordinator to join our team in supporting our patients andclinical staff at the surgery & PCN.We are seeking someone who enjoys working part of a team aswell as independently, who can multi-task and thrives in a busy workingenvironment.This is a varied role and a keen eye for detail isessential. Flexibility is important to be able to cover colleagues duringperiods of sickness or holidays.Training will be given in all aspects of the job, so thesuccessful candidate must be keen to learn new skills and enjoy a varied role.Candidates must be confident using IT.This is a full time role for 37.5 hours.
Main duties of the job
This role will involve a range of tasks including but not limited to:navigatingpatients to the right serviceassistingwith the patient recall processesBooking patients into clinicsInforming team when appointments slots are unutlisiedsignposting patientsassisting with general patient queriesmanagingmedical recordsreferralsdealingwith incoming clinical correspondence ensuring it is actioned appropriatelychasingclinical correspondences when test results are not yet returnedSupportin running extended access services (EAS)Openand close GP surgery.
