Job summary
We are looking for Care Navigator(s) to join our

dteam.To support thepractice clinical team by signposting patients to the appropriate healthcareprofessional or service, working as part of the practices multidisciplinaryteam. The care navigator will also be expected to undertake reception duties aspart of the role, supporting the administrative team in delivering a polite andprofessional service to the entitled patient population.Hours required: Monday 08:00-13:30, Tuesday 08:00-13:30 & Friday 13:15-18:45orTuesday 08:00-13:30, Wednesday 13:15-18:45, Thursday 08:00-13:30 and Friday 13:15-18:45.The hours required are non-negotiable.We are a training practice for trainee Doctors and medicalstudents, with a supportive team.Our Patient Participation Group (PPG) is very active andsupportive in the running of the practice.

Main duties of the job
Thefollowing are the core responsibilities of the Care Navigator.Process and signpost patientsManaging phones appropriatelyInitiate contact with and respond to requests frompatients, team members and external agenciesEnter read-code data on EMISDocument handlingManaging registrationsInput data into the patients healthcare recordsDirect requests for information to the appropriateteamManage queries as necessary in an efficient andpolite mannerCarry out system searches as requestedMaintain a clean, tidy, effective working areaMonitor and maintain the reception area andnoticeboardsSupport clinical staff with general tasks as neededParticipate in practice audit as directedSupport administrative staff, providing cover duringstaff absencesAction incoming emailsComplete opening and closing procedures inaccordance with the duty rotaSupport pharmacist(s) in the management of prescriptionsManage the movement of patient medical recordsAttend small group and team meetingsMonitor and replenish suppliesMedical Record Envelope managementSupport the surgery team to aid the smooth runningof the business
