Job summary
Cestria Health Centre has a fantastic opportunity for a Care Coordinator/General Practice Assistant (GPA) to join our team. The role is a support role, covering administrative tasks and, if appropriate, some basic clinical duties, to assist the GPs and the wider team, enabling them to make the best use of consultation time.The post-holder will form part of our multi-disciplinary, primary-care team and contribute to the smooth running of the surgery.

Main duties of the job
Manageincoming and outgoing communications (emails, documents, tasks)Helpwith appointment booking, patient queries and care navigationExplain treatment procedures to patients including arranging follow up appointmentsHelp people to manage their needs by answering their queriesand supporting them in making appointments andensuring patients have goodquality written orverbal information to help them make choices about their careMonitor referrals, missed appointments, investigations, to include cancer 2wwreferrals and missed follow up appointmentsRecord patient's demographic and simple health information as part of their personalised care plan/recordProvide patients with high quality, easy tounderstand information to assist them in making choices about their careSupport in the delivery of enhanced services and otherservice requirements on behalf of the PCNArrangeweekly practice meetings, including preparation of lists, creating teams linksand taking minutesManagingGPSI referrals, including scanning, registration of patients and organizingtriage, prior to appointment bookingWeeklyextraction of information from GPSI clinics to prepare datasets for submissionCompletingbasic (non-opinion) forms for the GP to approve and sign including, but notlimited to, insurance, mortgage and Department for Work and Pensions formsLeadon health promotion and education workTakeand record observations such as blood pressure, pulse, temperature

Qualifications
EssentialGCSE/NVQ level 2 or equivalent; or detailed patient services knowledgeDesirableQualified to NVQ level 3

Experience
EssentialIT confident .Must have customer/patient care skills and have worked in a customer/patient facing environment.Competence in Microsoft Office packages (Outlook, Word, Excel).Ability to compile and analyse statistical information.Desirable

Experience
of working with clinical systems i.e. Systm1 would be desirable.
