Care Navigator at the York Frailty Hub (Nimbuscare, Holgate YO26 4GG) is a hands‑on admin and phone‑facing role supporting some of the city’s most vulnerable, frail patients. You will answer incoming calls from patients and clinicians, schedule and amend appointments, update SystmOne and other systems, process documents and clinical rotas, and liaise with statutory and voluntary services. The post demands strict confidentiality, vigilance for safeguarding, a DBS check and the ability to travel between local sites. This is a full‑time permanent role, salary £25,896, with set days for weekend working; interviews are expected in York on 28 Oct 2025 and 4 Nov 2025.
