An exciting opportunity has arisen to join our Primary Care Network (PCN) as CHAW PCN Health and Wellbeing Hub Manager reporting to the CHAW Operational Lead and CHAW Clinical Director. The role is for 35 hours across 5 days working out of the hub in Wimlsow Health Centre, Chapel Lane, Wilmslow.
The hub has been running for over 3 years now and the successful candidate would be expected to further develop the existing services of the hub in collaboration with the practices. You will be responsible for the day to day management of the team and its services. You will also act as Digital Lead for the PCN. You will have extensive IT skills, exceptional organisational skills and have worked in a patient focused environment.
Job Purpose
The Manager will be responsible for the running of the CHAW PCN Health & Wellbeing Hub on behalf of the PCN. They will ensure that appropriate procedures and policies are adhered to and ensure the Hub is meeting its operational targets. They will liaise with the CHAW PCN Operational Lead and CHAW PCN Clinical Director to develop this service within the PCN. They will also carry out CHAW PCN management and administrative activities as required.
Job Description
As CHAW PCN Health & Wellbeing Hub Manager your key responsibilities will include, but not be limited to:-
General
Managing the CHAW PCN Health & Wellbeing Hub
Manage the team who will be working in the hub
Liaising with the team daily
Ensure they are working to agreed protocols and procedures
Manage staffing rota/holidays/absences
Acting as Digital Lead on behalf of the PCN
Be first port of call for the team for IT queries
Maximizing our use of available IT systems
Deal with initial HR queries in liaise with the CHAW PCN Operational Lead
Liaise with practices and other service providers regarding the services provided by the Hub
Ensuring we promote the services of the PCN to the practices and the patients
Be responsible for the quality outcomes of the service with the CHAW Operational Lead and CHAW CD Director
Ensure all procedures and policies are updated and reviewed on a regular basis and fully reflect our processes
Providing induction training on relevant IT systems and policies and procedures
Deal with patient complaints/SEAs/Incidents in collaboration with the CHAW Operational Led and CHAW CD Director
Learning Environment Facilitator
In collaboration with the CHAW PCN Operational Lead:-
Act as LEF for PCN
Liaise with Cheshire Hub regarding training initiatives available to the PCN
Maximise training opportunities for the PCN
Ensure appropriate funding is secured for the PCN
Work with GP trainer to help and support the development of ARRS staff
Ensure all staff have their relevant training
Digital/IT Development and PCN Website
Develop IT solutions within the PCN
Provide training support to all staff
Lead on items such as Emis Enterprise, GP Net, MS Teams, Apex, AccRx, Joy
Development and maintenance of the PCN website
To manage communications and social media pages to ensure patients have relevant and timely information to help them manage their health needs.
Research
In collaboration with the CHAW PCN Operational manager set up a research offer on behalf of the practices
Acting as the first point of contact for the research team
This is a new initiative and this role will develop
Health Equalities
In collaboration with CHAW PCN Operational Manager, and the PCN Health inequality GP develop the PCN Strategy covering:-
To lead on the PCN screening improvement plan, with the support of practices, to help improve cancer screening update and reduction of DNAs.
To take action to engage groups with low participation in chronic disease review, screening and immunisation, including childhood immunisations.

Knowledge, Skills and Qualifications
At least 2 years management experience in a health setting (ideally in general practice)
Experience of running a multi disciplinary team
Sound planning and organisational skills
Excellent IT skills
Excellent communication skills
GCSE grade A-C or equivalent qualification in Maths and English language, with high levels of literacy
